Friendly Visitor Program

925-692-0090 x 3 | christopher@rainbowcc.org

This program is designed to assist in reducing isolation for LGBTQ & HIV/AIDS older adults. The program addresses both the psychological and physical challenges seniors face living alone at home.  The Friendly Visitor Program is tailored to the needs of the individual, our process looks like this:

  1. We make contact with the referred senior and talk about services offered, we also set up their initial appointment with us.

  2. Their first appointment is attended by the senior program manager and a social worker to explore and identify individual needs.

  3. We follow up with future on-going appointments based on individualized needs.

  4. We encourage our senior to attend groups at Rainbow Community Center and/or other social groups in the community.

  5. We also help in accessing resources, such as:

  • Transportation

  • Food Resources

  • Healthcare

  • Housing

  • Social Resources

  • and more!

Our goal is to link seniors to resources and reduce isolation, for more information, send us referrals, or to set up an initial appointment please contact our Senior Program Manager, Christopher Holden at 925-692-0090 x 3 or email at christopher@rainbowcc.org
